<p>I submitted my transcript a week ago, but I did not include Form 7, the transcript request form. Should I submit another copy of my transcript? How crucial is Form 7?</p>

<p>Form 7 is not crucial at all. It is provided so you have something to sign and submit to your registrar to request the transcript. The form itself is not required by USC, just the official transcript.</p>
