Missing Teacher Evaluation Form URGENT!

<p>Hello!</p>

<p>I've had my letter of recommendation mailed by my teacher some time in November, but the Teacher Evaluation Form provided by the Common App wasn't included. My teacher told me that the letter included my name, and it's one that, well, no one has.
Anyway, the letter isn't showing up on Common App. I read somewhere that it will only show up if I invite the teacher online, but since my school uses Naviance this isn't possible.\
Is all well and good, or do I HAVE to send the letter of recommendation again, along with the teacher evaluation form? As in, is the teacher evaluation form required?</p>

<p>The teacher evaluation form is required, and if you’re not sure whether the college received the letter of rec, you should call the admissions office.
If you’re having trouble with Naviance then you should go talk to your counselor, who can tell you how to request teachers through Naviance. My school uses Naviance, and I had to sign into my Naviance account to request teacher letters of rec and link my Naviance account to my Common App account.</p>
