Need help ASAP - confusion over teacher rec forms

Hey all,
I am going to seek a recommendation from a Penn professor (I have just completed a class that he taught this summer). However, I am aware of the fact that Penn only asks for 2 recommendations. I also want to get 2 of my teachers from HS to give me letters too. Is it ok to submit more than form 3a and 3b? (as in, two of one of the forms then?)

I will be applying ED. What form should I give him (basically, what format/way should he write a recommendation letter?) Also, if anyone has any advice on if it's too early for him to actually send it in the undergrad admissions (given the fact that even if someone applies ED, it's going to be a while before they even submit their application to Penn), that'd be great. Thanks a lot.

if you have more than 2 read and only take the best two. The admissions board will probably read two random letters if you send 3+ letters

ask him to just write a supplemental letter instead of filling out one of the forms. but check first to see if Penn will read it.

What exactly is a supplemental letter? What information should I give him for this? And when would it be appropriate for him to mail it out?

what I"m going to do with my extra recommendations is get the prof to write the letter, put in envelope, seal it, sign it, give it to me...then when I apply, I'll just drop the sealed envelope inside the big manila envelope with all the other stuff.
