<p>The college official’s report form and the transcript are two separate documents.</p>
<p>yes, but the commonapp college official’s report form says “attach the applicant’s official transcript (check copies for readability)”. My advisor is confused whether it is necessary to attach yet another college transcript because the transcript has been sent from my college.</p>
<p>It looks like the want the 2 documents to come together (from the top of the Coll. Off. Rep):</p>

<p>However, since you’ve already sent the transcript separately, I’d advise you call your school(s) and ask them if they require a transcript in the same package as the Coll. Off. Rep. or if it can be sent separately. Colleges are accustomed to receiving pieces of an application in different mailings, so I don’t think it’s a problem, but you should give them a call to double check.</p>
