<p>They usually have a generic form which only asks for your teacher’s name and subject taught and etc. </p>

<p>They have a one page recommendation form that only asks for your name and teacher’s, or something like that. You should be able to download it from the application site. However, I emailed the admission office and asked if I could send in my letters of recommendation with the Comm App recommendation form, and they say it is okay with them. So, I sent in two teacher’s rec, and one counselor rec along with those rec Comm App rec forms.</p>

<p>I am pretty sure all require counselor recs. Usually you get a letter of rec from your counselor and attach it to the rec forms that can be downloaded from the application site for the particular university you are applying to. Some forms you can fill out yourself. If not, ask your counselor to fill it out and at least get a letter of rec from your counselor.</p>
<p>UC schools do not require teacher recs or counselor recs. You input your transcript yourself and they don’t look at any recs.</p>
