My school only offers yearly grades, we don't break things down into semesters. As I'm filling out my application though, there doesn't seem to be a way to note this. Should I just fill everything out as Semesters 1 and 2 but only put a grade after 2?

Couldn't you also put the same grade on both semesters 1 and 2. After all, you received that grade for your work both semesters.

Well the thing is, we do break it down into nine weeks and like, in German I had three A's and a B. Because the A's were low, though, it averaged out to a final B. I feel like I would be lying to say either A/B or B/B because the other As won't show up on my transcript, just the final B.

If I were you, I would either ask the College Counselor at your high school what previous students have done or find out the name of the USC Admissions Officer who is responsible for your high school and email your question to him/her. Anything one of us tells you could be wrong. I think you need an official answer.
