The health data form for new students is due June 1. I don’t think I can send it on time because I still have to go to the doctor to get it filled out. If it’s a few days late, are there any consequences?
There is a $30.00 late fee.
Assuming you are in generally good health, you don’t need to go to your doctor. My daughter had her questionnaire done at a “Minute Clinic” in a CVS drug store. You could also go to a walk-in clinic or urgent care clinic. However, you will need to provide your own immunization records to the school. If you don’t have the records, then you may need to go to your regular doctor to get them. (My information is from 2013, so it could be stale.)
