Omitting a College Transcript on Applications

So here's the deal, I took early admissions classes in highschool at my local CC and did well; however, after graduating I went to a private college in another state. I did awful while there, I really didn't give a crap and had a lot going on emotionally so I withdrew after 2 years with a 2.0 GPA. It's been six years and I've finally pulled myself together and actually figured out what I wanted to do with my life. I went back to my original CC and have a 3.6 GPA I didn't transfer my other university's transcripts because I was told that I didn't have to by a staff member I had a year to submit it if I wanted to. I'm finishing my AA next semester and I'm looking to transfer to another Private University in my state. They are asking for college transcripts, do I give them ALL of them? I do not want to jepardize my admittance or my GPA, but I also do not want to live my whole life looking over my shoulder worrying about it being considered being dishonest and thinking my B.S. will be revoked. I want to be a Physician's Assistant and I don't want it to affect my admission there either. Will some one please advise me?!

When applying for transfer you need to send all college transcripts, no matter when the courses were taken. Here is where colleges check your past college record:

[National Student Clearinghouse](http://www.studentclearinghouse.org/)

While submitting your old transcript may jeopardize your acceptance, not submitting will jeopardize your entire application, so it’s just not worth it. Also, since it was so long ago and you have shown obvious improvement, they are going to give that consideration.

Ditto what entomom said. Send all your transcripts, even if out of self-interest: a school can theoretically rescind your degree even after you’ve graduated if it comes to its attention that you falsified or omitted required application information. Unlikely to happen, but the expected value of successful admission weighed against the prospect of professional suicide comes up wanting - send everything.
