Optional Arts Supplement - Resume

<p>I'm wondering what an arts resume looks like..
Does it still have to have:
Firstname Lastname
Address something street...
City, province
Email
Phone</p>

<p>And then the content?
or just the content? But then, how would I title it?</p>

<p>Am I asking stupid questions..
Oh well, if I don't get it done I won't submit it because I'm applying to science...</p>
