<p>Does anyone know if MIT has a optional recommendation letter form? or do I just use the Evaluation A or Evalution B app. high school teacher form? I know my research professor at the university will def. write me a good one. (I'm doing a scientific research so I'm guessing Evaluation A)</p>
<p>Don't bother with either form, just have your optional rec writer write a letter covering the important points. Make sure the document includes your full name and birthdate on top, so they are sure to file it in your folder.</p>

<p>Assuming it's a letter from someone about an applicant, the Admissions staff is quite sharp and they'll snap right away to the fact that it's a supplemental rec. Your reviewer won't have to include a title. :)</p>
