My research mentor has written a recommendation letter for me. I am debating on whether he should mail it or use the common app's "Other Recommender" form. It may be more convenient to upload the letter using the "Other Recommender" form, but would my mentor have to fill out other questions about me, like my teachers have to? I want my mentor to just be able to submit his letter and not have take time to answer other questions.

No, he would not. He would just write how he knows you and for how long.

The “Other Recommender” form is different from the teacher evaluation form. Yes, there are questions to fill out, but they are not the same ones as on the teacher form. They do ask about the person’s relationship as well as about the recommender.

Ah, I see. Does anyone have a sample of this form or a list of the questions asked? I can’t find it anywhere online.

niceday, do you know how an “Other Recommender” can get the form to snail mail it?? Son’s teacher never got the invitation e-mail. School thinks it’s a Common App glitch.Counselor says to use snail mail. But how would teacher know what to send?

I’ll pm you.

@niceday @bridgemom having the same issue and urgently need to find a way to get ahold of this form so my other recommender can send in her stuff as the deadline is quickly approaching :(
