<p>You do not have to bring immunization forms to orientation. They will be mailed out the middle of July, which seems late since they require a dr. to fill out part. If you are going to orientation, you can pick up your forms rather than wait for the mail. They are in the Health Services building at 726 Broadway in the offices on the 4th floor. My daughter went last week and I went and got her forms.</p>

<p>I am positive you do not need the forms for orientation. The student affairs lady from Gallatin spoke to the parents and I specifically asked about the forms and she said they would be mailed in July. She made it sound like the forms just needed to be on file by the time students do their second registration for spring semester. I don't know if I would push it to that.</p>
