Personal Data Form?

<p>I am intending on applying to Georgetown as a transfer student for fall 2008; and from looking at the deadline/dates for the application process it says to "complete and mail back the Personal Data Form."</p>

<p>Where can I find the form? I've looked around the school website, but can only find one for freshmen applicants.</p>

<p>When filling out "Colleges previously attended" the form asks for "Credits Earned and Degrees Received."</p>

<p>Is this asking for the amount of units, and degrees accomplished by the time of transfer, or as of right now? And, in the case of the former, what if I don't entirely know how many units I will have completed, but more of a "ball-park" estimate +/- 4 units.</p>

<p>i would call the admissions office on that one if i were you, since they don't seem to have an faq on their website including that answer.</p>
