<p>That’s not part of the common app or supplement. Where are you seeing that term?</p>
<p>I asked an admissions counselor at CMU this question a day ago and here is the answer – this answer is geared for transfer students (as I am applying as one) but if you’re applying as a freshman, she covers that too – </p>
<p>“This refers to the Common Application form meant to be filled out by your college advisor or some other school official. It is similar to a high school guidance counselor recommendation, but for transfer students it is called the “College Official’s Report” by the Common App.”</p>
<p>If you are applying as a freshman, I think the “Secondary School Report” (CommonApp form) plus your high school transcript would suffice for when she says “high school guidance counselor recommendation”. </p>
<p>Any other questions, please ask. The list of requirements for students is a little vague and I had to get clarification on almost all of them.</p>

The Secondary School Report, Mid Year Report and Final Report are all part of the Counselor School Forms in the School Forms section of the Common Application. These should have been turned in by the student to their counselor at the same time the application was sent.
I’ve never heard them call “personal evaluation” forms before.</p>
