<p>I just got a letter from carnegie mellon saying my application is incomplete because my personal evaluation is missing...i didn't even know i had to do it! i found the link online but it looks almost exactly the same as the common app guidance recommendation. i'm wondering if this form is for those who didn't do the common app? when i check my status online at "Where am I in the process?" it doesn't even mention the personal evaluation.</p>
<p>The personal evaluation is the same as the guidance counselor form.
Everyone does the common app as that is the only thing CMU
accepts. I don't think they would care though if you sent the form
straight from the Common App or the one with the CMU supplement.
We had the same question re a teacher evaluation form and they said
either one of the forms was ok. You should check with your guidance
office and see what date it was sent (hopefully) and then call the
CMU admissions office and talk to someone personally about your situation.

<p>I did send in the guidance counselor recommendation and form for the common app and they got that...but they sent me a letter for the personal evaluation still.
The thing is... I never even knew this personal evaluation existed until I got the letter so I dropped the form off to my guidance counselor today and luckily she is making the time to fill it out tomorrow. I still plan on calling though...my guidance counselor seems a little confused about it also.</p>
