I just had a really quick question. Do you think that it would be too forward and bad if I made a sheet of personal qualities to give to my teacher when requesting a letter of recommendation? On this sheet, I plan to write like "I'm hardworking" and then kinda explain what i mean and list a few examples relevant to the class in which I demonstrated those traits.

What do you guys think? To do or not to do?

I'd say if you think your teacher will write a good rec, he/she should be able to write on your classroom experiences/participation. If you think your teacher doesn't know you well, then you should do it.

I don't think it's a good idea as it's up to your teacher, not you, to assess your personal qualities.

What would be helpful would be for you to give your teacher copies of any excellent tests or papers that you wrote for the teacher's class. Also include a note about your accomplishments in any activities that the teacher advises. Doing these things will help your teacher refer to specifics in her recommendation.
