PFAS Inaccessible?

<p>My PFAS form still doesn't work ..... It didn't work Jan 31st and I just tried again and it still gives me the same response: </p>

<p>You are not allowed to access the page 'chooseForm.jsp', if you are a member of another Group which is allowed, please select that Group. Restart the application

<p>Does anyone know what I should do?</p>

<p>^We downloaded the .pdf, filled it out and faxed it in. A lot of people had the same problem. You should do it today. They told me that the deadline was extended until February 7.</p>
