He told me to write an official document as a sort of proof that I really did shadow and observe him and he would sign it. please help, I have absoloutely nothing in mind. I want something that is kind of formal as this will be included in my application to med school...

<p>You technically dont need to prove that you shadowed someone. However, the next best thing is for them to write a LOR for you.</p>
<p>You don't need any kind of proof to apply for med school.</p>
<p>Also, one of the adcom members on SDN expressed the opinion (and I agree with this) that a LOR from a physician you've shadowed is practically worthless. They simply don't know enough about you from you following them around for a few days to recommend you for the responsibility of a physician.</p>
<p>then how else would you include the shadowing in your application?</p>
<p>on AMCAS, you simply have to list the activity, years and hours involved, a brief description, and an optional contact person - no strict proof is required.</p>
