<p>You have to imbed it in a word (.doc) document I think if you want to send it in online. But I'm not sure though... I just mailed mine in with my signature completion thing.</p>

<p>jpg and jpeg are the same thing. they don't want either. whoever said it above was right. they want you to paste it into a microsoft word document.</p>
