Assuming this info is correct:
sounds like you have a decent shot with EC's and awards, but your test scores are a little low. You seemed to do better on the ACT; since there is very little time left, maybe you should try retaking that? If you can raise your score by 1 or 2 points it would be a big help (work on reading). I don't even know if the dates for that have already passed, though.
All I can say is: apply! You never know. </p>
