<p>On the Common App, is the teacher evaluation a form or do they submit a letter? Should I give my teachers my resume and list of accomplishments in their class for the common app, or is that only necessary for letters that need to be sent on their own?</p>
<p>It is both a form and a letter (either attached or toured into a text box). And yes, give the teachers recommending you your resume.</p>
