PLEASE help me with a teacher rec!!

<p>I asked my English teacher to write me a rec letter for my common apps, and she did it right after I asked her (well, she promised to do it, and I assumed she did it, since when I gave her the thank you card, she seemed to have the expression that she did it)</p>

<p>I asked her around Sept. 15th.</p>

<p>But my counselor says that she she hasn't received a rec from my English teacher (she has received rec's from my other teachers though)...</p>

<p>How do I ask my english teacher without insulting/offending her?</p>

<p>Just go and drop by to say hi, the topic will move around to the rec and you can ask her.</p>

<p>But she's so extremely busy! O_O </p>

<p>She doesn't have any office hours (what she has she spends running around the school interrupting other classes to hand out papers). She's sort of quick tempered too...</p>

<p>To be frank, I'm a little afraid to aproach her u_u</p>
