the index for my state last year was 215 to 216, my index this year is 219. Am i basically guaranteed semifinalist?

Most likely as long as you have good grades and no discipline issues and you have confirming SAT score and you write the essay and submit the paperwork in time.
what do you mean by confirming sat score? @3scoutsmom I have a 1470 sat score also, is this a confirming score?
Yes 1470 should be good enough make sure you request that your score be sent to National Merit
Your PSAT score is all that will be considered to become a semifinalist, and you should be fine with your 219. To become a finalist, though, there’s an application process that looks at grades, confirming SAT score, an essay, etc. Confirming SAT score requirements will be set later by National Merit corp, and they will calculate a selection index from your SAT score the same way they calculate SI from the PSAT. I believe last year’s cutoff for confirming SAT score was a 209 SI.
If your SI is 219 and state cut off was 215 for yester year then congratulations. It’s highly unlikely that it would rise higher than 4 points.
