Question about additional recommendation

A minister at a church has agreed to write a reference letter for me, becuase she was impressed with me leadership and personal qualities (which I think would be beneficial b/c I was not very involved in school volunteering services).
I did an internship at her church and led the Sunday school as well as
an international group (community organization that welcomes new international and immigrant students and help them adjust to the new environment).

For additional recommendations, is it okay to just get her to write me one and seal it in an envelope with her signiture across the seal?
What kind of criteria should she use to evaluate me?
Are there any universities that forbid additional recommendations (competitive universities and LACs?)

Thanks in Advance.

Her letter needs to contain your name and some identifying information about you (ie school, address, etc), this can be up at the top of the letter.

And she can use whichever criteria she wants.

As far as individual universities, you really need to check each school's specific policies.

How about U Penn?
