question about applications (concerning teacher recommendations)

when suppplying teacher recommendations, is a letter of recommendation sufficient? or does the teacher need to fill out a form? for example, i am applying to carnegie mellon and they have a specialized teacher recommendation form. but it doesnt say anywhere on the site if i need to use that form. i would much rather just have my teacher write a letter and send it in with all my other materials. my question is, what should i do?

also, i have asked 3 teachers for recommendations, but most of the schools i'm applying to require just 1 or 2. is it okay for me to send all 3?

any advice would be appreciated.

Use the form, for most colleges it doesn't require any extra writing for the teacher, just checking off things. If the college wants a specialized form, give it to them.

does it matter if the forms are in color or black/white?
