Question about Common App "Midterm Report"

<p>Hi,</p>

<p>I've been looking for the answer to this question but I can't seem to find it anywhere.</p>

<p>Should I give each of my instructor a separate Midterm Report form (so 3-5 different forms)? Or should I just use one Midterm Report form and ask every instructor (whose class I'm taking currently) to sign and put the grade on the same form?</p>

<p>If I should just use one Midterm Report form and carry it around to different instructors, could we even waive our rights to the report, since we have to bring it everywhere and therefore have to have access to the report?</p>

<p>Help would be really much appreciated.</p>

<p>The form is meant to be taken around to all of your instructors and then mailed in. I’d just sign the waiver and not worry about it.</p>
