I forgot to fill out and mail it, and I have uncertainty about some information. I am an upcoming first-time freshmen. So for the “Where Living” box under the household information section, do I put where I am currently living or where I intend to live next school year?

@Ilyzium, after taking a look at the form, the “where living” question means where you intend to live during the 2018-19 school year. My apologies!
