<p>for the final high school transcript that we have to send in, is there any forms that we have to fill out? I know there was a form for the midyear transcript. and do we just have our school send in a transcript and proof of graduation in an envelope addressed to undergrad admissions?</p>
<p>Send it in with perhaps attach a letter with your name, UMID, uniqname.</p>

<p>I don't ever remember sending one in. I assume that my school did that for me, but I went to a school that basically sent in all my college stuff for me.</p>
