<p>There's several forms that as an incoming freshman I have to fill out. However most of them are in .pdf format. To whom do I send it these forms? Is there an email? Or I have to mail them?</p>
<p>It could not be any more confusing…but unfortunately, it’s the same drill this year as it was two years ago…
Some you fill out right on line,
Some you snail mail
and some are faxed / snail mailed</p>
<p>and some require someone else’s signature other than the student…so it’s a time consumer.</p>
<p>Most important-- fill in your housing and dining by the due date.
If you miss the housing form, you don’t get an on campus spot
If you miss the dining form (mail in or fax) they default you to the most expensive plan…</p>
<p>Everything else is not really time sensitive-- it’s preferred – but no serious consequences of any sort to be late…</p>
<p>You would think given CMU is a very technical school known for its great computer science that this process would be fluid and all electronic. Oh well…</p>
<p>^ I thought that too. But every year, the same questions and concerns occur with a small number of students. </p>
<p>I’ve come to a conclusion: No matter how well, how good, how worded, there will be the same number of students and probably the same type of students, who will have some type of problem problem in figuring it out. </p>
<p>Only suggestion I have, Don’t worry, Its not that complicated. 1,300 students incoming freshman don’t have issues.</p>

<p>Longtime…some people just aren’t good with administrative stuff…who knows why…</p>
<p>best advice for those confused…print out all the forms - all at once…they are all in one place after all…and then follow the instructions on each form…take you an hour tops…no big deal.</p>

<p>The medical record form when it becomes available needs to be returned to health services along with a copy of your immunization record.
In the meantime-- make sure your immunizations are up-to-date (see the list of required vaccines at the health services webpage) and get a copy of the record from your physician to mail /fax to CMU health services. Hep A, Hep B and menningitus aren’t commonly given in your early years-- and if an international, a TB test is required.</p>
<p>If you have allergies, diabetes or other medical condition that might impact your diet or room type, send in the medical forms that could in theory exempt you from a mandatory meal plan or make a modification in your room – such as no carpets, or install of an A/C (which you supply).</p>
<p>Also, final high school transcript showing diploma earned. That can come as late as mid July before they hunt you down. Just leave the form with your high school now with your signature.</p>
