Not sure where to post this so I thought I'd start here.

My son will be submitting a violin arts supplement. He was considering submitting a LOR along with it. He was going to ask the worship leader at our church (my son is worship violinist there) but now I see in both the Common App. and another college's art supplement app, they as for an arts teacher.

Should he not ask the worship leader? He's only been with his current violin teacher since March, so I'm not sure if she knows him well enough. He took a year off of violin, so his previous teachers recs would be too old, I think.

I suggest you have your son’s current violin teacher write the recommendation. The teacher, after 5 months of lessons and interaction, should have a good sense of his skills, character as a musician, and musical potential which I believe is the point of the requested letter for an arts supplement.

You may be able to submit both. Our daughter submitted two letters from two music teachers, along with a CD, to supplement her regular applications.

Well, being that the worship leader isn’t a teacher, I don’t know that it will add anything other than to show an area where my son exhibits talent and passion.

If the worship leader can shed light on an area where your son had talent and passion, it may be worth including his or her letter. You could ask admissions. I know my daughter was encouraged to include letters that rounded out the picture of her life. Teachers at school really didn’t know her as well as the music teachers outside of school, so we were told it was fine to send two extra letters of recommendation. It may depend on the school.
