<p>Where do you get the form for this? I know the Common App has one for high school students, but the transfer application doesn't. I already applied as a transfer to Emory, Lehigh, Bucknell, and Kenyon as of yesterday (yay!) and they require Mid Year Forms. Do you think making up my own form based off another school's form, including room for a signature from the professor and advisor would be a bad idea? I think I saw a Mid-Year Form for another college somewhere online, but I can't use it because it is specific to that school. Just wondering what everyone else was doing.</p>

<p>Lehigh has a mid-year report form, I know. I think you can download it. Or you can receive their entire Transfer app by mail. It comes pretty fast. The midyear form for them is called "Mid Semester Grade Estimate."</p>

<p>If the other schools require official forms, then they must provide it to you online or through the mail. </p>
<p>If you are just looking to submit mid-term grades you could do what I did last year:
- Ask your registrar or record keeping office to notarize and seal your mid-term grades and send it to the relevant schools (I did this)
or
- Ask professsors/advisors/TA to write a short note with your mid-term grade; compile them, copy them and send it out to the relevant schools</p>
<p>You can always call/email the Office and ask; it seems to be a fair question.</p>
