Question about teacher reccomendations

My college counselor told me today that I dont need to give the teacher rec forms from each college im applying to the teacher im getting my rec from. All he said my teacher had to do was write one letter of reccomendation and then just send it by itself to each school. Is this true? I know teachers usually just write one letter for every college, but are the reccomendation forms themselves a required part of the application?

I think they are. There are checklists on the CommonApp forms which are probably very important to individual colleges. I wish the teacher can just submit one to CommonApp and they will distribute to individual colleges, but probably that's not going happen (soon, at least).

I talked to an admissions officer from stanford yesterday. She says it doesnt really matter, but even if they dont fill out the rec form, they want it to be sent in anyway cuz it has this barcode on the bottom or something that just makes it easier for them to match up with my file.
