I just got my username and password today. Under "My Application Materials", it shows that they received all my "Basic Requirements".

However, in the section titled "Other Materials", it says "Additional Information(2)" and it has "1" next to "Number of items received". What could be missing? The "Other Materials" section also has "Penn Optional Essay", but it shows that they received that.

Also, is the "Mid-Year" report a form that gets mailed to us, or is it something that's available for download somewhere? We won't get 1st semester grades until January, well after the 12/15 ED notification date. Does that matter?

You'll only need to send in the Mid-Year Report if you are accepted or deferred. For accepted students I guess it's a way of making sure you're still staying on top of things. For deferred students, it's another element that will be evaluated in the spring.

how long did it take for you to receive the mypenn login info? like between the date you submitted your application and the day you received the info?

Well, kinda.
If you're accepted ED, then they'll look on your mid-year grades to make sure you're not slacking.
If you're deferred or apply for RD, you'll still have to show your mid-years, it's just that they'll be judged more harshly because they'll actually be a part of your evaluation criterion.
