<p>Is the counselor/teacher report suppose to be included inside the letter of recommendation envelope? in which case the paper would be folded...is that alright?</p>

<p>The usc website says to: Submit Form 4,Counselor/Teacher Report, with attached letter of recommendation (may be sent separately).</p>
<p>^ so my question is, am I suppose to have both the recommendation letter and the teacher report inside the envelope? I'm just confused because I've heard somewhere about how it's bad to fold forms, but in a standard sized envelope, the teacher form would be folded too if I put it inside...</p>
<p>I don't think this is unique to USC because other schools have teacher forms too, but USC is the one I'm looking at right now</p>

<p>On Form 4- Freshman Only, Counselor or Teacher Report- </p>
<p>You fill out Part 1, add your barcode sticker, and give it to the Counselor or Teacher (preferably with a stamped, addressed envelope) with the deadline clearly stated. The envelope can be small or large and it is ok to fold the forms.</p>
<p>The Counselor or Teacher fills out Part II of this form along with a "letter"- they refer to it as "comments" in question 6.</p>
<p>The Counselor or Teacher will mail it in separately from the other forms you send.</p>
<p>After you submit Part 1, you are supposed to receive a sheet of barcode stickers in the mail. These have your name and USC ID# and you are supposed to put them on every piece of paper you send them. Last year, we didn't get the barcode stickers and I called and asked what to do. They said to just write your USC ID# in that place or at the top right of every form. You should receive your USC ID# in the mail after they receive Part 1. If you didn't, call them and they'll give it to you over the phone. (We finally did receive the stickers.)</p>
<p>Hey danieldynasty- I got your answer. Fax in your application for a fee waiver. You will be able submit the online application and you will not have to pay.</p>
<p>^That's what the adcom told me today. I called for something else and asked your question and that's the answer I got. Maybe after you do it, you can come back here and post and let us know how it worked.</p>
