Question regarding spring grades for transfers.

Hi all, I'm a transfer student applying to Stanford University and Reed College, but I had just a generic question. I'd like to know if it is common practice for schools to make acceptance contingent upon spring grades for transfers.

I'm a learning purist, and though I claim my major to be "Physics," I study all areas. I take 18 credits minimum a semester, and I'm doing 23 credits this semester. I ran into some slight personal issues and my grades are suffering.

I have a 3.75 cumulative GPA with ~75 credits with all my classes 100 level and above, but I'm expecting this semester may hit me hard with a couple of B's where my transcript shows all A's and 3 B's way back when I started college. Back story aside, if anyone can answer my original question I would appreciate it.

Side question: my High School GPA is a 1.5, after which I did the Marine Corps for 4 years and am now about to earn an A.S. in Physics at a local community college. I took the SAT's last December without studying and did rather poorly on them - around 1910 if I remember correctly (I'm a naturally talented writer and my current math is at differential equations, but I didn't study for the SAT's and the tricks got me...I don't think that stuff was made for someone in my position/knowledge level). Does someone with experience know if they look at those numbers with any weight for someone with my circumstances? Thank you.
