<p>So, for the professor recs, do I just use the forms the common app provides for instructor evaluation and attach to that the teacher's letter of recommendation? I'm a bit confused. Also, one of our instructor evaluations comes from a high school counselor, right? </p>
<p>For Instructor LORs, first go to the college website and see how many they require, most are 2, but it varies. Also check for restrictions on who should fill out the form, usually it can be either a prof or TA.</p>
<p>You fill out the top part of the CA Instructor Evaluation form, then give it to your instructor to fill out the rest and either print their LOR on the form or attach it on a separate paper. You need to provide them with an envelope in which put everything and sign the back across the flap (for proof that it has not been tampered with). They can either return everything back to you to mail, or you can put the address and sufficient postage on the envelope when you give it to your instructor.</p>
<p>Instructor Evaluations are ONLY from college profs & TAs, not from HS teachers or GC. For your HS GC, you fill out the top of the Secondary School Final Report form, give it to your GC and they fill out the rest. Same holds with envelopes.</p>
<p>You also have to have someone at your college fill out the College Official’s form; this varies with the school, it could be your advisor, Department Chair, or some administrator designated to do this job. Ask your school whether they send it or if you pick it up and mail.</p>
<p>Also, I recommend using your return address for all of these forms, so that if they are returned for some reason, you can deal with the problem.</p>
