rec letter details

Just a few small, quick questions. My teacher said he wanted to do the recs by hand. I'm kinda desperate since I have to get my stuff to him by tomorrow (I know I suck for procrastinating).

1) What kind of envelope should I use?

2) What needs to be included on the envelope? Stamped and addressed, but do I include my high school's address as well? I'm not really sure where my teacher will mail it from.

3) Do I go ahead and print out 10 teacher evaluation forms? When the teacher packages them, do they go into the same envelope as the actual letter? Or should I get him 20 envelopes for the 10 colleges I'm applying to so he can mail the letter and evaluation forms separately?

4) On the teacher evaluation form, there is an "Evaluation" section that states: Please write whatever you think is important about this student, including a description of academic and personal characteristics, as demonstrated in your classroom. We welcome information that will help us to differentiate this student from others. (Feel free to attach an additional sheet or another reference you may have prepared on behalf of this student.).

^ Does the teacher have to fill out the evaluation section AND write a letter? Or is this the section that basically tells the teacher to write the letter. I imagine that if it is the former, a lot of the information would be the same, but I'm asking just to make sure.

5) How do I waive my rights? Is it just on the evaluation form or is there another place I can do it on Common App?

6) Is it okay if I have one teacher doing it via Naviance and one doing it via mail? Also, for the school forms on Naviance, I just invited the teacher. Do I also need to invite him on Common App to fill out the school forms? Actually I can't even find an invite option on the Common App. How do I do it and how will I know when he completed the online school forms and submitted the online rec?

Thanks!

Anyone?

I can answer some of your questions…

Use a business sized envelope, not sure the dimensions but it’s pretty standard, kind of long horizontally. Put one stamp on it unless you have many pieces of paper in there, in which case get it weighed at the post office. Put your school’s return address.

Sorry I don’t know much more… I’m not using Naviance or anything, my teachers are just printing out their letters and sending them via snail mail.

Regarding #s 3 and 4: Have your teachers fill out the evaluation form/checklist, attach the letter of recommendation to it and then mail together.
