Reccomendation Resume

How formalized should the resume you give to your counselor/teacher for a reccomendation be? My counselor asked for one, so I just quickly typed up a bulleted list--should I just list my activities with my name on the top, or should I be doing a full business style thing? Should I go into detail about my activities?

I searched the forums, but all I got was "don't forget to give your teacher a resume." How much detail should it have? A special heading? Help, please.

And just one more quick question: would it be forward to fill out on the Common App reccomendation I'm giving to a teacher what classes I had with him, even though that's the teacher's part? I just think he might have forgotten since there were several over the years and it seems less obnoxious than writing it on the resume. I think my school's teachers just write straight up letters and skip the forms so my handwriting on it probably won't matter.

Can anyone assist me?

For the counselor it doesn't have to be formal....points are just fine especially if they know you well. Otherwise you might go into the details that you excel in. Just remember most counselors don't have much time and if you give them tons of facts to wade through, they'll choke.

Teachers on the other hand should know you very well. Better to pick teachers that you've had within the last year first. Otherwise, pick teachers that know you very well. Remember too, that you want to pick people that will also give you a good review and who a excellent writers. English teachers are a good bet, so are science or math teachers.

You shouldn't have to give the teacher a lot of info if you've picked someone who knows you. Otherwise, you've picked the wrong person.
