Recommendation Forms Necessary?

Okay, so I screwed up. Being the clueless person that I am, I did not realize that there were specific forms for my teachers/counselor to fill out on the common app website. One of my teachers and my counselor already sent their letters of recommendation in. Fortunately, I realized my error before my second teacher sent her letter in and gave her the form (much to her displeasure). I'm also giving my counselor the correct form, even though she's already sent in her letter.

My question is, do the schools require the forms, or do you think the schools will accept the letters by themselves? Should I go through the trouble of asking my teachers/counselor to compile a new set of reccomendation information? I've already clearly upset one teacher by being clueless and not giving her the form, so I'm hesitant to ask my other teacher to fill out the form and send in a new batch of information.

If counselors write a letter that covers all the questions on the form, they can simply sign the form and refer to the attached letter. So the teacher you got the form to in time shouldn't be upset; tell her she can just do that and make copies of the form+letter and send one to each school. I don't know if you need to have the previously sent letters re-done. Some schools might care about seeing you sign the waiver, and some might not. It might be worth calling the colleges to ask what you should do.

i'm also interested to hear what you guys think about this. personally i think just the letter should suffice.

When my d applied, the teacher didn't send the form. I asked all the colleges if that was a problem; they all said no.
