but if a doctor who was my preceptor for the internship program writes a rec letter for me, does he need to fill out the "teacher evaluation" form out too??</p>
<p>I had a similar question because my professor from a summer program offered to write my rec. What I have been told is to either have that person fill out the complete teacher evaluation or to have them write only a letter and include it as a supplement in the “additional information” section. I would ask your guidance counselor, thought, to see if a doctor counts as a “teacher” for the eval if you want him to complete that part.</p>

<p>I am in a similar situation but I was told that it’s completely OK if that person writes the letter and sends it via snail mail. Is it true or does it have to be in the Common App?</p>
