Recommendation process...

A year ago, I asked for recommendations from 2 teachers; I ended up needing to transfer a semester later, so now I need to ask for them again. I just want to make sure I'm doing it in the best way - contact teachers to ask again (you know, make sure they're still alive), fill out my section and bring in the forms for each of the schools along with the old recc letter (so it can be easily rewritten in case they don't have the file saved), thank you visit after a few weeks to make sure they were sent.
Does that all sound appropriate? Did I miss anything?

Also, I'll use all common app teacher evaluation forms except for the schools that don't allow it (Cornell, Northwestern).

Remember to give them evelopes addressed to the school (and it's nice to include postage). A written thank you note is always a nice gesture.

Question: If I'm using the college specific transfer application that has its own teacher rec form, but the college also accepts the common app for transfers, can I use the common app teacher recommendation form instead of the school specific form (so my teachers don't have to fill out 9 different forms) while still using the school's application for everything else? In other words can I mix and match?
