Well, I'm applying to several different schools, and most don't use a common application, so the recommendation forms/formats are different. I have a few different teachers who have taught me in multiple classes and would most likely write a recommendation for me if I requested them to do so.

My question is - I don't want to overburden them, by having them fill out multiple forms/write recommendations, should I spread my recommendations out, even though one recommendation may be stronger than another?

Don't worry about overburdening them (unless you're talking about 15 schools or something nuts like that). They'll write on a word doc and cut and paste and print out what they need for each.

Most schools don't care if teachers use their form or another, as long as they get the information they need to evaluate you.

If you're concerned, call the admissions office of the non-Common App schools and ask if it's all right for your teacher to use the Common App form. I'm sure they'll say yes (all of my d's schools did).
