Recommendations from Work: Use the form?

<p>Hey CC,</p>

<p>I'm getting a recommendation from a manager of mine at the company I'm doing an internship with for my application to harvard, penn, stanford, nyu, and gtown. </p>

<p>I was wondering whether it would be best to have him just submit the letter or to also submit that "faculty reference" form they give you for the normal recs, those coming from professors. He said he would be able to talk about the quality of my work, my ambition, and dedication. What do you guys think?</p>

<p>Just have him send a letter on company masthead...that would be fine.</p>
