<p>I dont know if I just lost the second NW teacher rec form, but I only have one. Did the paper application originally have two in there? I printed a second one; at this point I'm beyond caring if they will want it on their own sheet...</p>
<p>On the back of the recs there is a sideways thing w/ Name, social security, and ID code. Do we fill that in or is that for office use? Thanks!</p>
