Regarding Readmission

I'm gonna hope that someone on this thread can offer some sort of substantial advice that can help me with my situation. I was admitted into New York University as an undergraduate of the class of 2012 back in 2008 right after high school. However, I went through a few nicks along the road and experienced some personal problems that started to affect my education and my motivation for it. I was dismissed after two semesters and started to see a therapist for my problems. After a while, I picked up on working here and there and, at the age of 20, moved up to the position of office and floor manager at my most recent workplace. I learned a lot of things during my absence from the world of academia and wish to pursue my education from where I left off; however, I am at a loss at where I should start and at which angle I should aim toward hitting. I'm planning to write an essay regarding my experiences and the lessons I have learned, along with having some recommendations sent along with my application. Anyone have any words of wisdom for my problem here?
Back in '08, I had a guaranteed admission offer to Cornell, but, because of my personal problems, I didn't get back to them on it, let alone my abysmal grades. Anyone see readmission to NYU for a semester or two, then jumping the gun to Cornell possible?
Thanks for reading through. I really hope one of you guys can help me out with enlightening me on something I've been struggling with in my mind for a while now.
