<p>This may be a dumb question, but it is advisable to put the school or home address (if the mail ends up being undeliverable and returned to the sender) as the return address for the Secondary School Report and Teacher Evaluations, even though the counselor/teacher will sign over the seal </p>

<p>Also, would it be bad if I write my name and date of birth on every page, or staple the pages?</p>
