ROL, Teacher Evaluation question

<p>I just want to know when a school asking you for a teacher evaluation. Is that just the form from common application?
Does my instructor only need to fill out the form and write some evaluation on the blank part at the bottom? Or does him need to provide an additional letter to accompany the evaluation form?
Or does him even need to fill that form out?</p>

<p>Or does him even need to fill that form out? He needs to fill out the Instructor Evaluation/LOR form.</p>
