Here is the template you should be using for frosh admission to SJSU, since much of the other template is irrelevant for SJSU admission:

Admitted: (yes or no)
Major applied for:
Major admitted to:
HS GPA, CSU weighted[1]:
SAT (CR+M superscored, no W):
SAT-based eligibility index[2] = GPA * 800 + SAT:
ACT (superscored without writing):
ACT-based eligibility index[2] = GPA * 200 + ACT * 10:
California resident: (yes or no)
Will graduate from a HS in Santa Clara County: (yes or no)

[1] https://secure.csumentor.edu/planning/high_school/gpa_calculator.asp
[2] https://secure.csumentor.edu/planning/high_school/elig_index_calculator.asp

Of course, the admission threshold eligibility indices by major for SJSU have been published here:
http://info.sjsu.edu/web-dbgen/narr/admission/rec-10079.12651.html
or linked from here if they regenerate the web site so that the above does not work:
http://info.sjsu.edu/static/admission/impaction.html

Assuming that your CSU-superscored ACT without writing is 31, then your ACT-based EI is 1056, which is higher than this year’s threshold for CS at 1021. This year’s threshold for SE is 862.
