<p>I'm not sure exactly what this form is. Do I just print off a copy of my scores off collegeboard or do I photocopy the reports I got in the mail? Or perhaps is it something else I'm supposed to attach to it? </p>

<p>You should ask collegeboard to send your SAT/ACT to USC directly. On top of that, you also should send USC a copy of your report. My daughter did both last year(with the suggestion from USC admission office). She received her admittance sooner than we were anticipated. I guess the sooner USC has your scores the better.</p>

That's right. Have the student report "officially" sent, but also fax your copy of the printout between the two pre-generated cover sheets. (We covered the schools that were printed on the report from College Board -- the freebies you get before you take the test -- just didn't think it would help to see UCLA, etc.)</p>
