<p>I found a copy, although I would only use it to start filling in information. I am pretty sure at least one question has changed since this version was released. Another, better, option would be to go to your local recruiting station. They can print one out that is the most up to date.

<p>The form is a big pain in the ass to fill out. It's used to start a background check for security clearances. As for when you fill one out, I know that USNA gave us the form with our permit to report so that we could fill it out later in the year.</p>

<p>speaking of forms: i took my prescription/ BCG form to my optometrist the other day, and they could only fill out a few blocks. they didn't size me for my glasses, like i said i needed. they said that the academy would have to fill it out because they could only measure my P.D. and everything else is based off that. anyone have similar problems? i figured they'd just measure what needed to be measured and circle the closest number on the sheet, but i guess there's a misunderstanding</p>
<p>Some people in my basic squad had to complete the BCG form on inprocessing day and they got to keep their contacts/civilian glasses for first beast. You definitely need those BCG's during second beast though.</p>
